Submitting PDF Forms From a Mac Computer

Thank you for taking the time to complete one of our submittable PDF forms. In order to submit a form on a Mac computer you must have the Adobe Acrobat Reader software installed. Unfortunately Preview, which is the default PDF viewer installed on Mac Computers, is unable to process submittable PDF forms.

Please follow the instructions below to install the needed software:

Click here to download the Adobe Acrobat Reader software.
Follow the on-screen directions to install the software on your computer.
Quit the Safari web browser if it's open.
Re-open the Safari web browser and navigate back to the PDF form you want to fill out. (Note: currently Firefox is not supported for Mac)
